We accept most medical insurance including Workman’s Compensation, Motor Vehicle Accidents, and Medicare. We will gladly call your insurance carrier to verify your out-of-network physical therapy benefits.

First Appointment Checklist

  • Early Arrival – 15 minutes: To maximize your treatment time, if you haven’t already completed your intake forms, please come in about fifteen minutes early for your first appointment.

  • Intake Forms: If you would like to save time on the day of your first appointment, we can email the intake forms to you.

  • Insurance Cards & ID: Copies of your photo ID and insurance card are required for services being paid by your insurance provider.

  • Doctor Referral: A doctor’s referral is not required to begin therapy. However, if your doctor referred you, please bring along their letter or therapy prescription.

  • Medical Records: Although it’s not required, it is helpful if you bring in any pertinent medical reports and diagnostics such as x-rays or MRI printouts.

  • What to Wear:  We ask that you dress comfortably or bring clothes that you can move around in easily. Shorts and a tank top are ideal.

  • Time Required: Please set aside an hour to an hour and a half for your initial consultation. During this time, the Dr. will discuss your condition and evaluate the extent of the injury.
